My Rat terrier has many (what seem like) health problems. What are they? What can I do?

I own an 11 year old, mid-sized Rat terrier. She is slightly overweight, breathes and snores heavily, throws up (more often the normal). Could you help possibly diagnose what may be wrong with her? Before I make a trip to the vet. Thank you!
